What the ten days are
The panel letter is the clock. The state’s process gives ten days to select from the three names on that letter. The claims administrator is not supposed to tell you which name to pick — that instruction is on the state’s own form, not a reading of ours. This page does not extend, pause, or restart that clock.
Form 105 is how an unrepresented worker requests a panel. Form 106 is the represented path. The evaluator you pick is not your treating doctor. The report is written for the claim.
Before the appointment
- Bring what the appointment notice lists. This page does not add to that list.
- The claim number and the panel letter are the identifiers the Medical Unit already used to generate the three names.
- The exam is a medical-legal evaluation, not a treatment visit. What happened at the evaluation is later argued from the report, not from this page.
